Pinellas County Library Co-Operative Card Reimbursement

 

If you have purchased a library card from a library part of the Pinellas County Library Co-Operative, you may download the form here. Simply mail the completed form along with the receipt from your library card purchase to the Town Hall at the address below.

Effective January 4th, 2008, the Town of Belleair will only issue reimbursements for library memberships for up to one year from date of purchase. The date of purchase should be indicated on the receipt that citizens bring in for reimbursement. Should the library Reimbursement Program cease to be funded, the Town of Belleair will not issue reimbursements regardless of date of purchase.
